HARMFUL PRACTICES DURING PREGNANCY

This afternoon whilst sleeping after my night shift, I had a strange dream. A woman in labor was on the floor trying to give birth. It was a difficult birth

The women attending to her placed her on the floor and one was standing on the abdomen giving fundal pressure so the baby would come out. I woke up wondering what was going on

It reminded me of my work in a rural area, where women go to traditional birth attendants to turn their babies who they claim are not in the right position. No scan or obstetric investigation done. The abdomen is manipulated until the women start bleeding as a result of placenta abruptio (The placenta separates from the walls of the uterus and the bleeding begins) . They come to the hospital in a collapsed state and also fetal demise due to the bleeding. They are only referred in the worst state when little can be done

Some get prolonged labor and are kept too long in the birth attendants home before the birth of the baby. This can result in obstetric fistula.

Fundal pressure is something even health workers do with so much caution. It does not have to be a routine.

Some are made to push through an undilated cervix leading to cervical tears which they may not be able to repair. They are only referred to the hospitals when complications sets in

Let me not forget giving herbal medications and concoctions to speed up labor which results in ruptured uterus and even fetal demise

It’s important that pregnant women seek skilled delivery to prevent complications that can cost their lives. Seek information from only skilled health professionals. Give birth in an accredited health facility.

Take steps to stay safe during pregnancy

THE GOLDEN MINUTE-NEONATAL RESCUCITATION

This refers to the first minute a baby is born into the world.

Once baby is born a quick assessment is done to ascertain the health of the baby. It is known as “APGAR SCORE”

It involves assessing baby’s appearance , pulse, grimace, activity and respiration. A score of 0 to 10 is given.

If baby has a low apgar or problem with respiration, activity or any of these vital signs, resuscitation is immediately started within the first minute.

Resuscitation can be as simply cleaning and stimulating the baby to using advanced methods to help baby breath. The lower the apgar the more advanced methods will be needed.

The earlier it is started the better. Delaying in initiating resuscitation can result in complications to the neonate, even brain damage.

Its important that before the baby is born the midwife prepares for resuscitation, regardless of the situation

It can be traumatic to both mum, baby and the medical team when baby is born needs some form of resuscitation and items are not available

In choosing your hospital this is one thing you should consider. How ready is the hospital for neonatal resuscitation. How skilled is the team to provide this if need be.

Resuscitation saves the lives of babies and very effective if initiated in the first minute

Neonatal resuscitation involves methods and techniques used to help your baby breath

CONDOM TAMPONADE- HELPS PREVENT BLEEDING AFTER BIRTH

For many of us we know a condom is used to prevent unwanted pregnancies and also prevent sexually-transmitted diseases.

In obstetrics it can also play a critical role. When a woman is bleeding after birth as a result of uterine atony. Meaning the uterus is not contracting properly, in a low resourced setting a condom together with other items can be used to form a condom tamponade

This is inserted into the uterus to help prevent the bleeding. It has been proven to be effective if done properly.

These basic things should be available in facilities for unexpected occurrences.

There are modern versions of this!

Kindly note that not all forms of bleeding after birth requires a condom tamponade. It depends on the cause of the bleeding!

AFTER BIRTH IT’S NOT OVER

Often than not once your baby is born you feel so relieved that you may forget to continue taking care of yourself

The first six weeks is the most crucial

You need to eat, rest, exercise, personal hygiene etc. Don’t forget your appointments

Failure to do so has landed lots of postnatal mums back to the hospital.

At least for the first six weeks consider yourself still recovering from the stress of birth. For some mums it even takes longer.

Take care of yourself and the baby also follow your doctor’s instructions.

If you had any condition during pregnancy, like diabetes, pre eclampsia, anemia, sickle cell etc. Please follow your doctor’s instructions even after birth. Don’t stop your medications or treatment because you have given birth until your doctor says so.

Beware complications can occur and be worse even after birth!

If you feel sick or have complications, report early.

It’s not over after your baby comes out. You need time to take care of yourself and fully recover!

OLIGOHYDRAMINOUS – REDUCED AMNIOTIC FLUID AROUND BABY

#MyBirthStory from @Offebs
I had a pretty much easy pregnancy from day one apart from poor appetite

Everything was going on smoothly until I got to 32 weeks where I was diagnosed with oligohydraminous (little fluid around the baby)

I guess it was as a result of me not eating and drinking properly and also 12 hour shift of work 5 times a week
I was put on a stick bed rest as well as an input and output chart

As at 33 weeks I couldn’t feel baby move properly so I went to take a scan which showed the fluid has reduced drastically and baby needs to be delivered

Fortunately or unfortunately I had 2 specialist taking care of me since am a health worker, (ie a specialist where I work and where I attend antenatal)

So it was the specialist where I work who did the scan and made an impression of an emergency c/s at 33 weeks ?? Oligohydraminous

I thought it wise to inform my gynea specialist where I attend ANC he made me come in quickly and accessed baby

Yes it was true that the fluid had reduced but everything about baby was fine besides as at 33 weeks she was underweight and also a preterm so together we decided to keep baby and be monitoring weekly to see and improvement and that’s exactly what we did

Everything seemed to be going on well weekly
So as at 38 weeks we decided to do an induction to deliver baby

So on Sunday, I got admitted and my labs were taken, baby was also monitored and everything was fine

Monday, 4am induction begun
Everything was going on well, I could stand the pain since I have a high pain stress hold I was dilating accordingly too

As at 9pm I was 5cm gone but baby’s head was down in pelvis which made me want to push with every contraction, so I was given spinal to numb the pain As at 12am Tuesday I was 8cm dilated but baby’s heart rate kept dropping

So we had to do an emergency c/s
At 1:15 baby was delivered with a weight of 2.7kg
Every other thing was fine

Mummy and baby are well

Advice: It can cause malformations, Infections, fetal demise and fetal distress. Report early when your water breaks. Cooperate with your doctor when diagnosed of oligohydraminous.
