DELIVERY OF THE PLACENTA (THE THIRD STAGE OF LABOR)

After the birth of your baby, it’s not over. The delivery of your placenta follows.

Within 30 minutes after birth the placenta should separate and be delivered.

It’s a very critical time and if not well managed can lead to serious complications like bleeding which is life threatening.

The placenta has membranes covering it, lobes that face the mum (maternal surface) and a smooth part that faces the baby. The cord has two arteries and a vein.

The midwife/ doctor has to inspect the placenta to make sure everything came out. There should be no missing lobe. The membranes should come out completely.

Any retained product can possibly prevent the uterus from contracting and cause bleeding.

Retained products can also get infected and cause problems several days later.

If after birth whilst in the recovery room or at home your bleeding doesn’t subside but you are actively bleeding call the team to re examine you.

Severe lower abdominal pains which are extreme after birth should also be reported for re examination.

Doing so in time can save your life!!!

HOW TO MAKE YOUR LABOR EASY

Labor has three stages

First stage.. Cervix which is the opening of the uterus dilates from 0cm to 10 cm. (Full Dilation)

Second stage… Cervix is fully dilated and mum can push baby out with midwives support

Third stage… Placenta is delivered

Contractions cause the cervix to dilate and it can be painful. Some few things you can do to ease the pain

🔴Walk around if your doctor confirms its ok to do so

🔴Warm showers

🔴Listening to music or engage in a healthy conversation with your support persons

🔴Sacral massage from your support person

🔴Assume a comfortable position. One that makes the birth easier. Learn about birth positions

🔴Eat enough if you are permitted to do so especially when no danger is anticipated

🔴Cooperate with your midwife. Always talk about your needs and how you feel with your midwife or doctor

🔴Request for pain relief. It should be in your birth plan

🔴Watch your favorite video or a healthy conversation with a loved one

🔴Dancing helps especially in the first stage of labor

🔴Don’t forget prayer works as well.

INTERVENTIONS SAVES LIFE- LETS END THE NEEDLESS MATERNAL COMPLICATIONS IN AFRICA

The midwife who helped to pay a mums bill sent a voice note from Obuasi.

#Story

The lady could not pay her bill but the hospital let her go home so she could come and pay little by little. She did caesarean section.

Due to her bill when she went home she never came for wound dressing. She was scared that when she comes to the hospital they will demand the money.

So when I shared that a mum wants to help someone, she called on her behalf and we paid it. She traced to the woman’s village to inform her that the bill has been paid and also check on her.

She realized the woman’s wound was getting infected since she could not come for dressing or postnatal. she wasn’t well. She sent her to the hospital immediately for the wound to be dressed. Also she will attend post natal for further care.

Yes we saved a life from complications. This could have ended differently. We thank God for the intervention!

This woman can freely walk to the hospital for care now. When she is well she can also take care of her baby.

This little things is what results in maternal deaths and complications.

The donor just saved a life using this platform and we are grateful to her. God richly bless her.

The midwife also did so well by tracing her to the village. Client had no phone and according to her even her living conditions is not encouraging. All the same we are grateful for this intervention.

PINK BABY- HEALTHY NEW BORN’SKIN COLOUR

The colour of your new born baby at birth should be pink

It shows that baby has enough supply of oxygen to all parts of the body

If the palm and feet are blue, it indicates a sign of cyanosis, a sign of reduced oxygen.

It’s more severe when the chest region is blue.

Call the attention of your doctor immediately

Report if your baby is blue, pale or yellow in color.

If your baby is dark in color and you cannot see the pink color, check the palm or the sole of the feet. It should not look pale or bluish.

RAPTURED UTERUS – AN OBSTETRIC EMERGENCY

To reduce maternal complications we have to talk about the problems we face in Africa

The uterus is the organ that carries pregnancy till birth

It is recommended that if you had a baby you wait for at least two years so it can regain its strength before the next baby especially with cesarian section.

Mothers with previous c/s who get pregnant within two years are likely to go through another planned c/s so they don’t go into labor since it can lead to a raptured uterus

Some mothers also take medications to speed up labor, when contractions is excessive it can lead to a raptured uterus. It’s important that you never take any medication to speed up your birth at home.

Women who have given birth to many children and also in short intervals also have a high risk of raptured uterus.

The risk is that the woman loses a lot of blood within a short time and can lead to fetal or maternal complications if an immediate cesarian section is not done to repair in minutes.

Speak to your midwife or doctor about your risk and how you can prevent it.

HOW DOES AN ECTOPIC PREGNANCY OCCUR?

The fetus is supposed to implant in the uterus where the blue arrows are pointing, but in an ectopic gestation the fetus rather implants outside the uterus. More frequently in the fallopian tube. The fallopian tube cannot accommodate a growing fetus. If not detected early it can lead to a raptured ectopic which comes with so much complications.

When you miss your period, confirm pregnancy by doing an early scan as your health provider the right time to do it.

Don’t wait for too long to start antenatal or join a pregnancy class.

At least they will be able to know if fetus is at the right place!

WHY CORD PROLAPSE IS CONSIDERED AS EMERGENCY?

🔴Umbilical cord prolapse is a complication that occurs prior to or during delivery of the baby. In a prolapse, the umbilical cord drops (prolapses) through the open cervix into the vagina ahead of the baby

🔴This is an emergency.

🔴If the cord is seen coming before the head of the baby and membranes are broken

🔴Blood supply to the baby can be cut off and you end up losing the baby or come with complications like severe asphyxiation

🔴If this happens at home, don’t spend a minute waiting.

🔴Move into a knee-to-chest position on the floor, with your bottom higher than your shoulders, to take the baby’s weight off your cervix. Stay in this position until the ambulance or until help arrives.

🔴Go to the hospital immediately

🔴Handling it unnecessarily can cause it to go into spasms which will cause more problems.

🔴At the hospital delivery will be done in the shortest possible time. An assisted delivery or an emergency Caesarian section will be done to save the baby.
