PERINEAL TEARS – WHAT YOU SHOULD KNOW

The perineum refers to the area between the vagina opening and the anus

During birth pressure from the baby’s head or presenting part can cause the perineum to tear on its own. It ranges from a 1st degree tear to a 4th degree tear which is more severe

Majority of the tears are first degree and can usually heal on its own

Third and fourth degree tears can result in fistula, post traumatic stress disorder among others

Some reasons for perineal tear include

🔴If it’s your first delivery.
🔴The position of the baby (face-up deliveries).
🔴Use of forceps or a vacuum during delivery.
🔴A large baby (more than 8 pounds).
🔴If you’ve had an episiotomy.

Some midwives anticipate the possibility of a tear and give an episiotomy instead to prevent complications

Some perineal tears can be mild and can heal even without repair. Some are more severe and needs repair

🔴Cooperate with your midwife
🔴Perineal massage
🔴Excercise during pregnancy
🔴Give birth at the right place with skilled professionals
🔴Choose the right birth positions

EXTERNAL CEPHALIC VERSION

Usually done for clients with breech presentation so baby can turn into the right position for vaginal delivery

It needs highly skilled person to perform procedure

It’s also done under strict monitoring and observation as seen in the video.

It’s sad that in some of our rural communities, women go to untrained persons when they are told they have breech presentations.

They do this procedure without any monitoring device.

Clients end up having complications like placenta Abruptio, shock and subsequently death.

If diagnosed with breech, ask your doctor or midwife the position and exercise you can do to help the baby turn into the right position.

Avoid dangerous procedures that can lead to serious complications

Also cooperate with your doctor when it comes to your mode of delivery

MY DUE DATE CHANGES ANY TIME I TAKE A SCAN

When it comes to your due date, the most reliable is your first scan taken during early pregnancy.

Late scan are not reliable for determining your due date

Even if you have different scans with different due dates, the first scan is what your doctor or midwife will use in determining the maturity of the pregnancy

If you know your last menstrual period, then that is even more reliable. You will have to remember the first day of your last menstrual period.

Taking note of your due date is important for various reasons.

It guides you on when to expect your baby. The doctor will also know when to intervene to prevent issues of post date etc

OBSTETRIC FISTULA – A BIRTH COMPLICATION THAT CAN BE PREVENTED

Birth brings joy to most women and their families, but this is not the same for every woman. Some are left with complications that affect their quality of life permanently.

One of such complications is what is termed as an obstetric fistula. A fistula is an opening that occurs between two organs. In the case of obstetric fistula, there is an opening between the vagina and either the bladder or rectum.

So the woman may leak urine and feces through the vagina. She has no control over it. Imagine what happens to these women. They are left in isolation and their partners sometimes abandon them due to the incontinence. Low self esteem becomes a part of them. They may never go for public events due to the smell around them. It gives way to infections and other health challenges.

This is caused by prolonged and obstructed labor. Imagine a woman being in labor for three days! There is reduced blood supply to the organs and the tissue dies with time. An opening is then created between the two organs.

The good news is that with skilled birth most obstetric fistula can be prevented. Deliver in a hospital. If it occurs it can also be corrected in most of our tertiary hospitals by surgery. There are women who are affected but are not willing to talk about this due to stigma and financial difficulties.

Let’s encourage our women to give birth at facilities that provide skilled delivery and also provide emergency obstetric care.

Midwives should manage all births skillfully using the patograph as a guide. Refer promptly where necessary to prevent some of these complications.

The sad part of this is that most of these happen in the developing world, especially rural areas where our Healthcare system is not that strong! Educate your loved ones about this! #Share

PAY ATTENTION TO YOUR DUE DATE DURING PREGNANCY- THE PLACENTA

This looks like a beautiful tree there

Yes its the placenta what connects the mother and baby in utero

Its responsible for exchange of oxygen, nutrients etc

Without it your baby will not grow properly

Always want to remind mums that at a point during labor if you have not given birth the placenta can stop functioning, function improperly etc leading to fetal distress or still birth

Always be mindful of your date of delivery and work closely with your doctor or midwife around that time

Your pregnancy should not go beyond 42 weeks!
