MY ABDOMEN IS NOT AS BIG AS I EXPECT- 1ST PREGNANCY

This is because it is your first pregnancy and the abdominal muscles are still tight.

It hasn’t been stretched before. It will not look as big as a mum who has delivered before.

So you don’t have to worry or compare your abdomen to another person’s pregnancy

In second or third pregnancies because the abdominal muscle has been stretched before there is already some amount of laxity in the abdominal muscles and so it can grow bigger than that of a first time mum

Also remember other things like fluid, fat or any mass can make the abdomen look bigger.

So you don’t really need a big abdomen to feel all is going well.

Your doctor or midwife will also be checking on your fundal height as well as your scan. This will give your doctor insight on the well being of the baby. If he has not complained that you have intrauterine growth restriction, you should be at ease and enjoy your healthy pregnancy!

WHAT CAN I ADD TO TURKEY BERRY TO GET MORE IRON?

Turkey berry is widely known for its benefit of helping to boost your hemoglobin level due to its rich source of iron.

You can take it as a juice, add to salads or soups etc

To get the iron absorbed by your body, it’s advisable to add orange juice or tangerine juice. This is rich in vitamin C and helps in the absorption of the iron. Other fruits rich in vitamin c can also be used.

Eat turkey berry in moderation and make sure you prepare it under hygienic conditions.

Ask your doctor or midwife if its ok for you to consume

A nutritionist should also teach you how much is enough so you can be safe.

CAN MY WATERS BREAK BEFORE LABOR SETS IN?

Every baby is beautifully enclosed in membranes surrounded by water or amniotic fluid during pregnancy. This protects the baby in utero. It should not break until labor sets in.

Yes the membrane around your baby can break before labor sets in, when this happens it means the water around your baby can leak or come out. No matter what weeks you are once your water breaks you need to report to the hospital immediately.

This is an emergency.

If you wait too long an infection can set in. Your baby can also go into fetal distress.

Report immediately for the best care to save your baby.

Don’t leave your pregnancy to chance.

MOULDING AT BIRTH

Sometimes we wonder how a baby’s head is birthed during delivery. This is what happens

The fetal skull is made up of bones which are flexible, soft and has spaces between them

The flexibility allows the bones to overlap(as in the picture) so that it can pass through the pelvis and the birth canal

After birth it takes its normal shape with time but still has spaces in between the bones. This also allows brain development.

The skull hardens after the brain is fully developed

You may realize your baby after birth has two spaces or fontanelles on the head. One in front and the other at the back

It will close naturally by itself. You don’t need to apply anything to it.

Some problems that can occur during the birth process to the skull are caput succedaneum and cephalohematoma. It resolves with time. Follow your doctor’s instructions and Cooperate during birth for the best outcomes.

WHAT IS THE BEST SLEEPING POSITION FOR BABIES

The best sleeping position for your newborn or baby according to research is for the baby to lie on his back with head tilted and not on the side or abdomen.

Sleeping on the side or abdomen increases the possibility of #SuddenInfantDeath. Babies can roll over or get suffocated easily with these positions.

Bed should be firm and the sheets should be fitting. Do not leave toys or items in your baby’s bed especially at night.

Breastfeeding has also been found to help reduce Sudden Infant deaths. Breastfeed baby especially during the first six months.

In cases where the baby is vomiting or sick keep a close eye on the baby. Child may aspirate what he is vomiting and can lead to complications or sudden infant death.

At the hospital the doctor/nurse may use his or her discretion as to what position will make the baby safe. If a child has the tendency of aspirating due to vomiting or secretions, he may recommend a position that favors the baby’s health.

11 WAYS TO PREVENT HEADACHES DURING PREGNANCY

Severe headache during pregnancy could be a sign of stress, hunger, poor ventilation, dehydration, malaria or an indication of pre eclampsia .

It could also be caused by hormonal changes during pregnancy.

Anemia or low Hemoglobin levels can also cause headaches

To avoid headache,

🔴Rest in-between your activities,

🔴Take in enough fluids, water, healthy drinks, soups etc

🔴Take your malaria prophylaxis as ordered to prevent malaria

🔴Always check your BP and urine protein at the clinic if headache is severe

🔴Your doctor will rule out the specific cause and treat accordingly.

🔴Take prescribed analgesics

🔴Regular exercise

🔴 Eat on time and food should be nutritious.

🔴Proper ventilation to reduce stuffiness

🔴Eat on time and in adequate proportions

🔴Also take your daily dose of prescribed Hematinics

If headache is persistent and doesn’t go away with rest, report immediately for further investigations and treatment. It’s a danger sign of pregnancy

HARMFUL PRACTICES DURING PREGNANCY

This afternoon whilst sleeping after my night shift, I had a strange dream. A woman in labor was on the floor trying to give birth. It was a difficult birth

The women attending to her placed her on the floor and one was standing on the abdomen giving fundal pressure so the baby would come out. I woke up wondering what was going on

It reminded me of my work in a rural area, where women go to traditional birth attendants to turn their babies who they claim are not in the right position. No scan or obstetric investigation done. The abdomen is manipulated until the women start bleeding as a result of placenta abruptio (The placenta separates from the walls of the uterus and the bleeding begins) . They come to the hospital in a collapsed state and also fetal demise due to the bleeding. They are only referred in the worst state when little can be done

Some get prolonged labor and are kept too long in the birth attendants home before the birth of the baby. This can result in obstetric fistula.

Fundal pressure is something even health workers do with so much caution. It does not have to be a routine.

Some are made to push through an undilated cervix leading to cervical tears which they may not be able to repair. They are only referred to the hospitals when complications sets in

Let me not forget giving herbal medications and concoctions to speed up labor which results in ruptured uterus and even fetal demise

It’s important that pregnant women seek skilled delivery to prevent complications that can cost their lives. Seek information from only skilled health professionals. Give birth in an accredited health facility.

THE GOLDEN MINUTE-NEONATAL RESCUCITATION

This refers to the first minute a baby is born into the world.

Once baby is born a quick assessment is done to ascertain the health of the baby. It is known as “APGAR SCORE”

It involves assessing baby’s appearance , pulse, grimace, activity and respiration. A score of 0 to 10 is given.

If baby has a low apgar or problem with respiration, activity or any of these vital signs, resuscitation is immediately started within the first minute.

Resuscitation can be as simply cleaning and stimulating the baby to using advanced methods to help baby breath. The lower the apgar the more advanced methods will be needed.

The earlier it is started the better. Delaying in initiating resuscitation can result in complications to the neonate, even brain damage.

Its important that before the baby is born the midwife prepares for resuscitation, regardless of the situation

It can be traumatic to both mum, baby and the medical team when baby is born needs some form of resuscitation and items are not available

In choosing your hospital this is one thing you should consider. How ready is the hospital for neonatal resuscitation. How skilled is the team to provide this if need be.

Resuscitation saves the lives of babies and very effective if initiated in the first minute

CONDOM TAMPONADE- HELPS PREVENT BLEEDING AFTER BIRTH

For many of us we know a condom is used to prevent unwanted pregnancies and also prevent sexually-transmitted diseases.

In obstetrics it can also play a critical role. When a woman is bleeding after birth as a result of uterine atony. Meaning the uterus is not contracting properly, in a low resourced setting a condom together with other items can be used to form a condom tamponade

This is inserted into the uterus to help prevent the bleeding. It has been proven to be effective if done properly.

These basic things should be available in facilities for unexpected occurrences.

There are modern versions of this!

Kindly note that not all forms of bleeding after birth requires a condom tamponade. It depends on the cause of the bleeding!
