VAGINAL EXAMINATION DURING LABOR

One main concern women have during labor is vaginal examination. Some have asked whether something else can be done to substitute vaginal examination during labor.

It’s one of the procedures that just can’t be avoided once you are In labor as it’s significance can’t be underestimated. It can be done using a speculum or the fingers, sometimes both. There are key roles it plays. Vagina examination allows your midwife or doctor to check the following

🔴Vulva and vagina to make sure it’s can allow vaginal delivery. No sores, rashes, female circumcision etc

🔴Cervix-It’s dilation

🔴Membranes- Whether it’s intact or not

🔴Liquor- If the water around baby is clear r meconium stained

🔴Presenting part- Which part of the baby is coming first

🔴Pelvis- Whether it’s adequate or not

It is an uncomfortable procedure and can be a little painful. Your doctor or midwife will prepare you by informing, seeking your consent and reassuring you before its done

Its done usually after an abdominal examination. It is not done except it is really needed as frequent vaginal examination can result in infection if care is not taken. It is done under sterile conditions. During labor it is usually done every 4 hours and more frequently when it is very necessary.

Your midwife will communicate all finding to you after the procedure. Cooperate with your doctor or midwife for the best outcomes.

In some cases vaginal examination may be contraindicated. Example. Complete placenta previa previa.

YOUR HEMOGLOBIN(HB) LEVEL BEFORE BIRTH

Hemoglobin (Hb or Hgb) is a protein in red blood cells that carries oxygen throughout the body. Normal range is between 12 to 16 g/dl for adults

During pregnancy #hemoglobin is needed for the growth and development of your baby. In the mother low hemoglobin leads to anemia which can lead to further complications like hemorrhage during birth.

During pregnancy your HB is expected to be within a high range for your safety and that of your baby.

HB of 11g/dl or more is recommended. 10 g/dl is safe but even in this case you will be encouraged to boost your HB before delivery sets in.

Hemorrhage/bleeding is one of the causes of maternal mortality in #Africa.

Having a high HB makes you more safe during delivery.

In case of any complications because your HB is even high, emergency procedures can be started earlier to prevent further complications.

🔴Always take your prescribed hematinics to keep you HB high.

Eat well, take foods rich in Iron, red meat, milk green leafy vegetables etc

Take your prescribed dewormer

Know that if your HB is very low blood transfusion may be requested before your delivery or to keep your pregnancy healthy

If you have any condition like sickle cell, G6PD etc talk to your doctor and request to be seen by a specialist.

If your #HB is low 7, 8, 9 ask your midwife what you can do to increase it. You can speak to a #nutritionist as well.

I CAN’T FEEL MY BABY MOVE- IS MY BABY ASLEEP IN UTERO?

Its important that once quickening which is the first baby movement a mum feels during pregnancy starts, it should not stop or slow down

At least you should feel your baby move every day

Mums should be familiar with how their babies move and if there is any significant change report immediately.

Your baby’s fetal movement should not slow down or become excessive.

If you cant feel it at all kindly report to the hospital.

Never assume its because your baby is sleeping!

Your baby’s movement can start between 16 to 22 weeks of pregnancy. It can be earlier for mums who have given birth before.

Other factors may affect how you feel the baby’s movement. But do report if it slows down or becomes excessive

Excessive baby movement or decrease in baby’s movement pattern could be a sign that baby is in distress.

MAKING FOOD CHOICES ESPECIALLY DURING PREGNANCY

Went for a walk with my kids, as we were coming back home they asked that I buy fried plantain popularly known as “KELEWELE” in Ghana .

I said no. I love it, but not after taking a long walk to burn calories and then just come back home and replace it immediately is not the best.

We finally bought watermelon which is a healthier choice for the night. Less fat, less calories, more water and a good amount of roughage. Easily digestible and has vitamins and minerals.

Its the same with every day life and pregnancy. The choices of food you make will determine how healthy you will be.

It will determine your weight gain during pregnancy as well

Healthy choices have to be a conscious effort

You should know what works at what time you should take it

Talk to a nutritionist if you need help

IS IT NORMAL TO LEAK BREAST MILK DURING PREGNANCY?

Breastmilk is the first and perfect food that nature provides for babies. It’s referred to as natures perfect food and golden.

It’s important to know that breastmilk starts to form during pregnancy. It does not wait for baby to be born first. The first milk is called colostrum and this is already formed before the birth of the baby

Mums in their third trimester of pregnancy when they express breastmilk will realise that some liquid comes out. Sometimes yellowish or clear liquid. This is perfectly ok. The body is preparing for the baby

This is why you should trust your body and start breastfeeding immediately the baby is born. Don’t assume there is no breastmilk. There is breast milk. All you need to do is to allow baby to suck more. The more the baby sucks the more breastmilk is produced.

Don’t rush to give formula. The more you give formula the more the baby will be satisfied and will not suck . This can cause reduced breastmilk supply and also nipple confusion

Trust in your body and ability to breastfeed and give baby on demand .Breastmilk has many advantages for both mum and baby.

Let me repeat that your breastmilk leaking during pregnancy is very normal and doesn’t come with any risk. If it’s bloody, brown, greenish, comes with pain or skin changes report to your doctor for prompt assessment.

CERVICAL DILATION

CERVICAL DILATION

Your cervix which is known as the mouth /opening of the uterus during pregnancy is closed. When labor sets in it will begin to dilate

Your cervix should be fully dilated before you push out your baby. Pushing through an undilated cervix can cause cervical tears.

Always cooperate with your midwife. Push when she has confirmed that the cervix is fully opened.

Having severe pains does not mean that cervix is fully dilated. Let your midwife confirm first!. Unless baby comes out spontaneously don’t be in a rush to push without it being confirmed.

And remember cervix is dilated when you are 10 cm full. Its a gradual process that can last between 8 to 18 hours. Its shorter for mums who have given birth before.

DRESSING YOUR NEWBORN BABY

Most newborn babies are born with a perfect skin, well hydrated no rashes and having good skin color

Unfortunately some mums will return to the hospital after few days with their babies having skin rashes, dehydrated skin and sometimes jaundiced

One factor that contributes to the poor skin turgor in babies is not dressing the baby appropriately according to the weather

In Africa where we have a hot climate, it’s important to consider the weather when choosing your baby’s clothing

One clue to help you dress your baby is that if you feel cold, your baby is probably feeling cold as well. If you feel hot baby is probably feeling hot as well

Choose cotton dresses that are more comfortable for the baby. Dresses worn by babies should be dry. A wet dress can cause hypothermia or decrease in temperature

Baby should be properly covered. Clothes made from Cotton is preferable. Just one layer which suits our climate. Socks and a cap. Wrapped nicely in a cot sheet. Don’t add many clothes. It’s not needed with the weather we have here in Africa, Ghana.

Don’t wrap the baby with too many clothes. It can cause overheating leading to further complications. It also causes skin problems like heat rashes etc

Babies dress should be washed with mild soap and also rinsed with lots of water to prevent babies skin from reacting with the chemicals in the soap.

Not to forget, with our weather here in Ghana please just wrap baby with a cot sheet and that is enough. Except weather is very cold and it demands extra clothing

Continue to breastfeed exclusively for six months and avoid using harsh soaps on the babies skin.

Baby dress should be comfortable and not too tight on the baby

A baby who is not comfortable will be fussy and may cry as well

During cold weather make sure the baby is well covered, socks and a cap with probably some extra clothes to prevent hypothermia.

It’s important to maintain a clean environment and the room well ventilated. Report to the hospital if your baby has a high temperature or the body is extremely cold.

PREVENTION OF STILL BIRTHS

❤️Effective monitoring of all pregnant women to detect possible abnormalities
and correcting those abnormalities. Monitoring, recording, and interpreting
fundal height rightly to detect growth restriction.

❤️Screening of patients to rule out infections that can cause growth restrictions
and possible treatments.

❤️Health education on nutrition, personal hygiene, danger signs, rest and
exercise, etc to promote healthy lifestyle among pregnant women.

❤️Taking prescribed drugs

❤️Providing skilled delivery for all pregnant women to prevent and also
reduce the risk of having a prolonged labor which can cause still births.

❤️Preconception care to expectant mothers and their partners.

❤️Performing fetal kick counts starting at 28 weeks of gestation and also being conversant with your body. In recent time its recommended that you will be familiar with your baby’s movement. Report if its excessive or reduced.

❤️Calling your midwife when you experience any unusual symptom or sign

❤️Taking nutritional supplements to reduce the risk of chromosomal abnormalities, example folic acid.

Talk to your midwives or gynaecologist as well. Remember most of these still births are preventable.

CERVICAL TEAR-A PREVENTABLE INJURY

Some times you wonder why you are still shouting, having all the pains and your midwife is still asking you to hold on!

She checks and says, it’s only 1 cm dilated.

After a long wait or 4 hours, she rechecks and says it just 3cm.

You feel it’s been too long and the pain is unbearable so you should push your baby out.

This is why you should cooperate with your doctor or midwife.

The cervix which is the opening of the cervix should open from 1cm to 10 cm to allow the baby’s head to be birthed followed by the body.

Cervix should not be felt any more when your doctor or midwife performs vaginal examination.

If it’s still felt and you push through it, you can tear your cervix which is difficult to repair and requires special skills. Bleeding can also be severe.

Only push when your doctor or midwife has confirmed that you are fully dilated. (10 cm and no cervix can be felt on vaginal examination)

MY BIRTH STORY – C/S ON ACCOUNT OF FAILURE TO PROGRESS

My EDD was 23/08/2024
My blue jet decided he wanted to ‘be outside’ earlier and arrived on 03/08/2024
I had quite a bearable pregnancy I’d say. I felt like I had terrible malaria between weeks 6 and 16.
After that I got almost all my mojo back but for some dizziness and extreme fatigue that surfaced during the 3rd trimester. And oh I could sleep. I neither experienced vomiting nor spitting.

On the 25th July I experienced some bleeding and rushed to the hospital. It was a bit more than a ‘ bloody show’ so drs suspected a number of things and said I ll be admitted till baby comes but after thorough assessment and scans I was discharged the following day with the assurance that I wasn’t in danger.
For some reason, I always felt baby will come at 37/38 weeks so I told hubby that after week 37 no traveling or staying at work for long😂 I guess it was my instincts kicking in.

On the dawn of 02 Aug, I woke up to my underwear completely soaked. Initially I thought it was urine but I kept leaking and upon assessing the fluid I concluded my water had broken. I called my doctor and mw Sally and they both said I should report immediately.
I arrived at the hospital at 2pm and when I was checked I was 3cm but I was surprised cos I felt absolutely no pain and wasn’t contracting. After about 6 hours, still no contractions and I was given medicines to augment the contractions. Then came the most painful physical pain I’ve ever experienced😄. After more than 24 hours, and still leaking liquor with baby’s heart rate going higher than normal, I was taken in for a cs on account of failure to progress. In a few mins my baby was out and I smiled hearing his cry.

Somewhere deep within me I was mad at myself that I ended up at the theatre to the extent that I’ll skip anything that had to do with svd on social media and dealing with the pain post cs made me angrier. But I had to pep talk myself that it wasn’t my fault that things happened the way they did, and that God saved my life and my baby and that’s what matters most.
